Office Policies

We look forward to seeing you on your scheduled appointment date. Our office may contact you a few days prior to your appointment to confirm the appointment date and time, pre-register standard information and remind you of the upcoming appointment. When checking-in, it may be necessary to obtain copies of insurance ID card(s) in order to bill your insurance company.



Free 15-minute Consultations

These free initial visits are available by phone. This consultation allows us to briefly get to know each other, your mental health needs and services that can be offered. This free service does not guarantee future care, and does not establish a client/provider relationship. If appropriate after consult, a full intake assessment will be scheduled.  

Intake and Follow Up Appointments

Following the free consultation, a full intake appointment can be scheduled either by the provider or patient via the patient portal. The frequency of follow up visits will be determined at a cadence we agree on. It depends on factors such as the severity and complexity of your situation, current symptoms, the types of medications prescribed, and the availability of additional support from sources like family, friends, or therapists. Visits may range from monthly or more frequently initially, until you achieve stability and improved well-being, to quarterly visits once all necessary reports and assessments are up to date. In line with widely accepted community care standards, clients receiving psychotropic mediations must be evaluated by a medical professional at least once every 90 days to continue their medication regimen. Failure to attend an evaluation within this timeframe may require a follow-up visit to assess medication needs.

Payment

A current form of payment, either debit or credit card, must be placed on file to schedule your appointment. If you utilize health insurance, our office will process your payments. If you are a self-pay client your payment will be processed via the patient portal.

Late Arrival Policy

Arriving late (more than 15 minutes) for an appointment may lead to cancellation or rescheduling.



After-Hours Emergencies

Please be aware we do not provide crisis management services. If you are experiencing suicidal or homicidal thoughts, are in crisis, or need immediate help, please call 911 or go to the nearest emergency department. We do not follow patients in the hospital or see patients while hospitalized. When hospitalization should occur/become necessary, the inpatient treatment facility or hospital typically provides an in-patient psychiatrist. Once you have discharged from your higher level of care, whether it be inpatient, partial hospitalization, intensive outpatient program or residential care, please make an appointment with our practice for continuation of treatment.

Cancellation or No-Show Fee

We kindly request a minimum of 24-hour notice for appointment cancellations or rescheduling. This allows time to allocate the appointment to someone on waitlist, ensuring efficient care for all. Clients who cancel an appointment or fail to attend with less than 24 hours' notice are subject to a $25 cancellation or no-show fee, this applies to both new patients and established patients. Please be aware that you hold financial responsibility for appointments not rescheduled or canceled with appropriate advance notice.

Provider Cancellations

Every attempt will be made to inform clients in advance of planned provider absences. If there is a need to cancel an appointment at the last-minute, we will reach out as soon as possible and reschedule or have a member of my staff connect with you.

Missed Appointments

Repeated instances of missed appointments without prior notification may result in discharge from practice. This decision aims to ensure that we can offer care to all patients effectively.
